Medical Records- The personal information in your record may be seen by various staff to provide services, case reviews, billing, customer services, etc. The staff may need to use the information in your record in order to successfully carry out their duties. Be advised however that all staff members are also held to a strict confidentiality agreement that is legally binding.

Appointment Reminders- We may email you to remind you of your next appointment.

This involves the sharing of information, which requires your signed consent.

Additional Services- in the event that it is jointly determined that you may benefit from the services of another agency/service provider, sharing certain information about you may be necessary. Some examples of such agencies are schools, hospitals, lawyers, nursing homes and courts.

Other Persons Involved in Your Care- It may become necessary to share some information with people or agencies that are helping us to provide your services. These may include family members, friends or anyone who helps you, for whom you have given us consent.

Your Medical Doctor- Because it is important to coordinate your mental health care with your physical health care we may request your consent to share information with your doctor.

Imprisonment- It may be necessary to release some personal information to the prison authorities if you are an inmate of a prison.

Revoking Consent- You may revoke your consent but you must put it in writing.

Section 3: Mandatory Disclosures
This involves the sharing of information which we are required by law and ethical dictates to divulge. In such instances we are unable to protect the privacy of your personal information.

To Avert a Serious Threat to Health or Safety of Self or Others- We will share personal information about you to stop a serious threat to your health and safety, and to the health and safety of others to whom you may be deemed a threat.

Law Enforcement- We may have to release personal information if requested by law enforcement officials in the following instances:

b) If required to assist in the locating or identification of a suspect, fugitive, material witness, or missing person.
